The MN-F file is a general medical necessity crosswalk file comprised of CPT or HCPCS/ICD-9-CM codes pairs derived from Medicare local coverage decisions, national coverage decisions, and commercial insurer medical necessity policies. The file was developed to cover the largest number of medical services possible, currently about 3000 procedures, services, pharmaceuticals, and equipment. Overall, the use of the file should be a first pass check to determine whether there are payers who have determined that a particular service should have a particular diagnostic requirement. There are inherent limitations with the file, and you should be aware of the best way to utilize the file.
