EZClaim Basic Report List and Descriptions

• Aging Report - Summary report showing all claims that have not been marked as paid. Groups by patient group. Shows the aging of the claims based on the bill date (located on the charges screen). Can be filtered by Facility and Patient.
• AR Report by Carrier – Aging Report grouped by Carrier.
• Billed Procedures by Facility – Claim List report grouped by facility name and procedure.  Only shows claims that have been exported. If a claim has an exported date showing the charges screen, it will show up on the report. Can be filtered by Service Date, Facility, and Patient.
• Carrier Mailing Labels – Label report uses Avery 5160 labels. A carrier label will print for every claim that has a first date of service between the date range entered on the reports screen. Can be filtered by Facility, Patient, and First Claim Date.
• Claim List – Summary report showing claim charges and total amount paid. Grouped by Facility. Can be filtered by Service Date, Facility, and Patient.
• Claim List by Carrier – Claim List report grouped by Carrier.
• Claim List by Group and Carrier – Claim List report grouped by Patient Group and Carrier.
• Claim List by Group and Referring – Claim List report grouped by Patient Group and Referring Provider.
• Claim List by State – Claim List report grouped by patient state.
• Claim List Detail – Detail report showing each date of service, claim charges and total amount paid. Grouped by Facility. Can be filtered by Service Date, Facility, and Patient.
• Claim Notes - Report showing claim notes.  This report is used when printing a claim note from the Notes window. Can be filtered by Facility and Patient.
• Daily Summary – Claim list detail grouped by date. Shows the amount billed or collected each day. Can be filtered by Service Date, Facility, and Patient.
• Income Statement - Shows charge, insurance payment, co-pay, and adjustment subtotaled by patient. Can be filtered by Service Date, Facility, and Patient.
• Paid Claim List - Shows total charges, insurance payments, patient payments, and adjustments. Can be filtered by Facility, Patient, and Bill Date range. When the report is run, you must enter the Start Bill Date and Ending Bill Date. The Bill Date field is found on the Charges screen. If the Bill Date field is blank, the claim will not show on the report.
• Patient and Claim Notes – Shows all patient and claim notes. Can be filtered by Facility, Patient, and First Claim Date.
• Patient Birthday Mailing Labels – Prints labels for all patients born in a selected month. Can be filtered by Facility and Patient. When the report is run, it will ask for the month to print.
• Patient Detail w Visits – Shows patient name, address, and ID information. Also includes dates of service and totals. Can be filtered by Service Date, Facility, and Patient.
• Patient List – Prints name, ID, account number, phone number and active status. Can be filtered by Facility and Patient.
• Patient List by Group – Patient List report grouped by patient group name.
• Patient List by Plan Name – Patient List report grouped by insurance plan name.
• Patient List Detail - Shows patient name, address, ID information, and insurance information. Can be filtered by Facility and Patient.
• Patient List w DOB and ID – Prints name, ID, DOB, phone number and active status. Can be filtered by Facility and Patient.
• Patient Mailing Labels – Print patient name and address using Avery 5160 labels. Can be filtered by Facility and Patient.
• Patient Notes – Prints patient name and both the EDI notes and Patient Notes. Can be filtered by Facility and Patient.
• Patient Procedures by Facility – Grouped by facility, patient, then procedures. Can be filtered by Service Date, Facility, and Patient.
• Patient Unit Count – Patient List showing the total number of units (or days). Can be filtered by Service Date, Facility, and Patient.
• Patient Visits – Patient name, service dates, procedures, and charges. Also counts the number of unique days the patient has been seen. Can be filtered by Service Date, Facility, and Patient.
• Patient Visits by Facility – Patient Visits report grouped by Facility. Can be filtered by Service Date, Facility, and Patient.
• Patient Visits by Plan Name – Patient Visits report grouped by insurance plan name. Can be filtered by Service Date, Facility, and Patient.
• Patient Visits by Procedure – Patient Visits report grouped by Procedure. Can be filtered by Service Date, Facility, and Patient.
• Printed Claim List by Carrier – Claim list showing patient names and services grouped by insurance carrier. Can be filtered by Printed Date, Facility, and Patient.
• Printed Claims – Claim list showing patient names and services. Can be filtered by Printed Date, Facility, and Patient.
• Printed Claims w Diag – Claim list showing patient names, services, and diagnostic codes. Can be filtered by Printed Date, Facility, and Patient.
• Prior Auth Number by Procedure – Shows service date range, units and charges grouped and subtotaled by patient, authorization number and procedure code. Can be filtered by Facility and Patient.
• Prior Auth Number Usage - Shows service date range, units and charges grouped and subtotaled by authorization number and patient. Can be filtered by Facility and Patient.
• Procedures by Facility – List of services, units and charges grouped by patient and facility. Can be filtered by Service Date, Facility, and Patient.
• Referring Provider Patient Count – List of referring providers, their ID’s and the number patients and claims. Can be filtered by Service Date, Facility, and Patient.
• Services without Patient Payment - Detail report showing each date of service and payment detail. Only shows claims that have not been marked as paid and dates of service that do not have some type of patient payment. Shows total charges, insurance payments, patient payments, and adjustments. Can be filtered by Facility and Patient.
• Services without Insurance Payment - Detail report showing each date of service and payment detail. Only shows claims that have not been marked as paid and dates of service that do not have some type of insurance payment. Shows total charges, insurance payments, patient payments, and adjustments. Can be filtered by Facility and Patient.
• Statement – Patient statement listing all services on claims not marked as paid. Can be folded and mailed in standard windowed envelope. Uses the providers name and address (Box 33 on the 1500 form) as the return address. Shows service date, procedure code, description, charges, break down of payments and balance due. Can be filtered by patient.
• Statement w Aging – Statement report with claim aging totals at bottom of page.
• Statement w Aging and Diag – Statement report with claim aging totals at bottom of page. Also includes the diagnostic codes (without descriptions).
• Statement w Diag – Statement report with diagnostic codes (without descriptions).
• Statement w Facility – Statement report with diagnostic codes (without descriptions) using the facility name and address as the return address.
